ATR Ohana Application Forms will be accepted from January 9, 2012 through February 9, 2012, from providers currently delivering services to individuals with substance abuse issues on the Island of Kaua’i. If you are interested in applying to be an ATR Ohana provider from the Island of Kaua’i, please print out and complete the two (2) page Application Form (SEE LINK BELOW), and scan the completed form to: JoHanna.Mechergui@doh.hawaii.gov, or fax the completed form to (808) 692-7521, Attention, Dr. JoHanna Mechergui, ATR Service Developer.
The deadline for submitting new applications for the Island of Oahu was on September 6, 2011. No additional applications are currently being processed for agencies proposing to deliver ATR Ohana services on the Island of Oahu.
